We invite you to take part in a research study about the knowledge, attitudes, and beliefs of American Indian and Alaska Native youth related to underage substance use.
If you’re interested in filling out our 20–25 minute long survey, you’ll receive a $25 VISA gift card upon completion. By participating in this research, the knowledge you share will help inform substance use prevention projects centered on American Indian and Alaska Native youth.
To qualify, you must:
Be 9–25 years old (if under 18 years old, your caretaker will need to give permission for your participation).
Self-identify as American Indian and/or Alaska Native.
Live in Seattle/King County, WA, or receive care from any urban Native organization in King County, WA.
This survey is completely optional, and you can opt out and/or ask questions at any time. Opting out will not affect your ability to seek and receive any form of care at the Seattle Indian Health Board. If you need any additional resources, download our resource card below.

Survey Flow for Participants
Step 1. Scan QR code on Flyer: This will take you to a blogpost (this one). Here we’ll determine if you’re eligible.
Step 2. Sign Pre-Survey Forms: RedCap Link on blogpost. If you’re under 18, a caretaker will also need to sign the form with you. Be ready to provide your phone number or email. Once your consent forms have been reviewed by our staff, you will be provided the link to the survey via text or email.
Step 3. Take the Survey: This will take between 20 to 25 minutes on average to complete.
Step 4. Receive Gift Card: You will have a chance to confirm your preferred delivery method and address. Also, you’ll receive a post-survey completion confirmation email.
